Comments: Received permit application and check number 2232 for $260.00 for permit fee from Elnathan Davis at the time of the reinspection. We will have to get a CE200 completion sheet to get a CE200 filled out by us. All violations from the inspection report dated 12/20/2018 have been corrected CORRECTIONS 1H- No venison or other food from an unapproved source found on site. The product from last time was removed by the owner's friend. 2C- No no chicken wing liquid observed leaking, and no improper storage observed. No risk of contamination. 8A- All bottles labeled for their contents. 10B- Bare wood surface in the walk-in cooler that had chicken wing juice on it has been removed and replaced by a new piece of wood. The wood has been painted, owner will be adding an additional coat of paint to it. 12E- Both paper towel dispensers working properly at this time. 14B- No opeeen traps seen on-site. 15A- Thr cracked and broken floor tiles have been replaced. 15B- Fan grates have been defrosted.
