In hot box- Meat sauce 173, cheese 168. Sanitizer noted at 200ppm or greater. Over flowing floor drain previously noted at 3 bay sink not observed at time of inspection. Third quarter testing is to be completed during the months of July, August or September. Also as a reminder annual nitrate testing is required prior to the end of 2017. Ehrlich was contracted to treat a problem with millipedes in the last month receipt received at inspection.

Comments: Temperatures adequate throughout: diced tomatoes 35 degrees; hot holding rice 148 degrees; ground meat in hot box 145 degrees; walk in cooler 38 degrees. Received June and July Water Operation Reports from manager at time of inspection. Due to General Manager being gone for at least two months, she was unaware that they had to be sent in to DOH. August is being recorded daily and discussed need to send in the operation reports in a timely manner upon the close of each month. UV system operating at time of inspection. Fly complaint appears unfounded-no flies or other pests seen in establishment at time of inspection. Discussed complaint with manager who was aware of situation and is monitoring the business daily for pests. Complaint pertaining to restaurant being unclean in general is accurate, as seen in violations noted during this inspection. More frequent and thorough cleaning of all surfaces is necessary. Mail report also.

Comments: Temperatures taken of food products and in refrigeration were found to be acceptable. Intensity meter for ultraviolet water treatment systems is observed at 82 %. Proper glove use noted. New RGM coming in on Wed, 12/21 first name is Brittany last name not known to staff.

Comments: Temperatures taken in refrigeration and of food items were found to be acceptable. Current intensity meter reading for UV treatment system is 85 %. Water operation reports for the UV treatment system have not been submitted to NYSDOH office for the months of October, November and December 2015, January 2016 report is due today 1/10/2016. NYSDOH office also has not received a copy of the results for required anual Nitrate testing. No operation reports are available to view at the time of inspection. Dialy monitoring of water treatment is required by all food service on private well and operation reports are required to be available during inspection. Submit test results and missing operation reports immediately. New RGM Bobbie Baker took over location in the last week. Please mail inspection report to facility address.

Comments: Discussed the deficiency with the operator. Also discussed hot and cold held items with the operator. The following Air temperatures were measured (degrees F): Walk-In Cooler - 35; Bain Marie - 42. The following Food Temperatures were measured (degrees F): Upper Compartment Bain Marie - Diced Tomatoes - 37.5 and Hash - 43.6; Steam Table - Taco Meat - 171.0, Scrambled Eggs - 143.6, Steak Strips - 156.7, Sausage Patties - 185.4, Refried Beans - 164.9, Chicken Strips 148.9, Shredded Chicken - 158.8. The Fire Suppression System was checked in November 2014, the Sanitizing Bay had a Quat Residual of 300 ppm, and the Sanitizing Buckets for the wiping cloths were about 200 ppm. The UV Sanitizing System for the potable water supply was on and all the lights were "green". The well can be checked after the snow melts since it was now covered by snow and inaccessible.
