Received June and July Water Operation Reports from manager at time of inspection. Due to General Manager being gone for at least two months, she was unaware that they had to be sent in to DOH. August is being recorded daily and discussed need to send in the operation reports in a timely manner upon the close of each month. UV system operating at time of inspection. Fly complaint appears unfounded-no flies or other pests seen in establishment at time of inspection. Discussed complaint with manager who was aware of situation and is monitoring the business daily for pests. Complaint pertaining to restaurant being unclean in general is accurate, as seen in violations noted during this inspection. More frequent and thorough cleaning of all surfaces is necessary. Also discussed hot and cold held items with the operator. The following Air temperatures were measured (degrees F): Walk-In Cooler - 35; Bain Marie - 42. The following Food Temperatures were measured (degrees F): Upper Compartment Bain Marie - Diced Tomatoes - 37.5 and Hash - 43.6; Steam Table - Taco Meat - 171.0, Scrambled Eggs - 143.6, Steak Strips - 156.7, Sausage Patties - 185.4, Refried Beans - 164.9, Chicken Strips 148.9, Shredded Chicken - 158.8. The Fire Suppression System was checked in November 2014, the Sanitizing Bay had a Quat Residual of 300 ppm, and the Sanitizing Buckets for the wiping cloths were about 200 ppm. The UV Sanitizing System for the potable water supply was on and all the lights were "green". The well can be checked after the snow melts since it was now covered by snow and inaccessible.
