Observed all time and temperature control for safety/potentially hazardous food (TCS/PHF) in hot holding or cold holding (no room temperature storage observed). Observed an employee placing one piece of bread back into bag with bare hand. Interview with employee indicated that bread is either toasted or placed on grill. Discussed the requirement to cook/toast bread to 140F if bare hand contact occurs. Discussed the requirement to wear gloves after hand washing to prevent contamination with ready to eat food items. Manager stated he would provide training to all staff regarding glove/utensil use.

Observed sanitizer (Chlorine) bucket at 100ppm per facility's test strips and 100ppm in 3 bay sink. Observed digital thermometer at cook line and thermocouple outside of manager's office. Reviewed policy for waivers on pancake batter, raw shell eggs, and liquid eggs with label for 4 hour discard time requirement.

A Time Table of Compliance Date of March 25, 2016 has been established to correct remaining violations listed above. A re-inspection will be conducted after this date to determine compliance.
