Comments: Reviewed temperatures logs with General Manager (Brandon Merkle). Cold hold and hot hold temperatures are recorded two times daily. Buffalo Exterminating provides monthly services at facility. No evidence of rodent or insect activity observed at time of inspection. Discussed sick food worker policy. Provided hand wash stickers at time of inspection. (2)

Comments: Received complaint per phone call on 12-29-2016 in regards to food served on a dirty plate (dried egg material) on 12-28-2016 at approx noon. Made field visit to restaurant. Spoke with manager Brandon. Examined approx 20 clean plates/bowls at cook station. Found 3 bowls and 1 plate with dried food residue on them. Manager removed them from stack and placed them in dirty dish bin. We then examined dishes coming out from commercial dishwasher. The dishwasher uses a low temperature final rinse with sanitizer. We found 2 plates and 1 bowl with food residue left on them. Manager will have a serviceman come in to look at dishwasher asap. In the meantime he will have his staff be more observant during rinsing operation prior to washing, and have his staff look closely at dishes prior to serving food to customers. Manager will contact Health Dept after dishwasher has been serviced. Made follow up appointment for late next week to reinspect.
