All glue boards have replaced and clear of pests at this time. Ceiling leak has been addressed and stained ceiling tiles have been replaced. Some (fresh) droppings noted in kitchen and storage areas. Future discussion and meetings with Rachael Murphy and Dave Delaney to be scheduled by this office to determine long term solution to pest problems.Entire hood cleaning scheduled for Christmas break per food staff.

Comments: A re-inspection on this dates reveals accummulation of rodent droppings (along baseboard) in several locations within the kitchen area as well as in the rear hallway between the loading dock and garbage storage. Effectiveness of the current pest control program cannot be evaluated until all affected are adequately cleaned. Old and fresh droppings musted be removed so that the areas can be monitored for new activity.

Comments: Reinspection notes the above remaining violations from the April 4, 2017 and May 3, 2017 inspections. A three month abatement schedule has been given to address the above remaining violations. A follow up visit will be conducted beginning/mid August to assess the progres with correcting those violations. There still appears to be some rodent activity in the dry storage room. Shelving and window ledge noted free of any droppings but there are still droppings noted on the floor mostly under the wooden shelving, along the base board floor edges. Continue daily checks, monitoring, and cleaning of the dry storage room to gage activity. Maintain pest extermination. Recommend a thorough cleaing of the dry storage room when the food product level is lower and the shelving can be easily moved to access hard to reach areas. Please call with any questions, 435-6617.

Comments: Reinspection notes the above remaining violations from the April 4, 2017 inspection. An additional reinspection will be done in approximately 2 weeks to access the rodent activity. Salt City Extermination contracted weekly; invoices on site from April 11, April 19, and April 25, 2017. Kitchen manager states that the dry storage room has been cleaned and that the mouse droppings noted today are new. Maintainance is scheduled today to begin cleaning the shelving in the dry storage room, and will also pull the shelving away from the wall and clean the floors under the shelving, as well as any areas along the base boards that are noted with mouse droppings (maintainance vacuuming dry storage room at this time). Continue daily checks and monitoring of the dry storage room for activity. Clean and sanitize any unopened food items that mouse droppings are noted on and discard any open food items where mouse droppings were noted on.
